Registration
This form library contains standard forms used by the school for regiustration and various other purposes. Their use may be subject to special requirements or restrictions. Please call the school if you have a question. New Registrants: To apply for entrance, you will need the following for each child: A completed School Registration Form (one per family). A completed Student Information Sheet. A Birth Certificate. If Catholic, a Baptismal Certificate. A Health Immunization record. A $115 Registration fee. New registrants should call the school for an appointment (908-276-9425) and bring the required forms and check to the school. Note that all Registrations are subject to the Admission Policies and Requirements which are described in the School Handbook) School Registration Form This form must be completed for each new family registering. School Re-registration Form This form must be completed for each returning family registering. Student Information Sheet This form must be completed for each child registering. Other important forms: Student Emergency Information Sheet This form must be completed for each child registering and returned on the first day of school. History & Physical Form This form must be completed for each child registering and returned to the school on the August 1 preceding the academic year. After School Care Registration This form must be completed for each family registering a child or children in the St. Michael School After School Care Program. Before School Care Registration This form must be completed for each child registering in the St. Michael School Before School Care Program. Media Permission Slip This form must be completed for each family registering. In Town Trip Permission Slip This form must be completed for each family registering. Parent Permission Form for Field Trips This form must be completed for out-of-town field trips. Family Fundraising Commitment This form must be completed by each family with a child in any of Grades K through 8. HSA forms: Scrip Order Form This form is used to order Pathmark and Shoprite scrip.
